Summary

  • Sesamoiditis presents as a severe pain under the 1st metatarsophalangeal joint.
  • Common in dancers and sports people.

Diagnostic tips

  1. Tender sesamoid bones
  2. Pain intensifies on dorsiflexion.

Tests and Imaging

  1. X-rays of forefoot
  2. Bone scan, if the X-rays are normal

Immediate Treatment

  1. NSAIDs
  2. Choose appropriate shoes: low-heel and soft-sole
  3. R.I.C.E.R.

Possible Referral

  1. Podiatry for footwear advice, padding and splinting, possible orthoses.
  2. Sesamoidectomy after failiure of conservative treatment
